About This Role
The Case Manager, Registered Nurse - Field position at CVS Health involves collaborating with members, providers, and community organizations to address the health care and social needs of dual eligible populations. This role is ideal for individuals who are independent thinkers and possess strong analytical and communication skills.

- Location: Marion, IL

- Residency: Must reside in the state of Illinois
- Travel: Up to 50-75% travel required in Williamson, Jackson, and Union Counties, IL
- Utilize critical thinking and judgment to inform the case management process.
- Facilitate appropriate healthcare outcomes for members.
- Assist with appointment scheduling and accessing benefits.
- Collaborate with members, providers, and community organizations.
- Use care management tools and resources effectively.
Education:
- Associate's degree required, bachelor's preferred
- Minimum 3-5 years clinical practical experience
- Preferred 2-3 years in Care Management, discharge planning, and/or home health care coordination
Certifications:
- Active and unencumbered Registered Nurse license in the state of Illinois
- Certified Case Manager preferred
