• Associate's degree required, bachelor's preferred

• Minimum 3-5 years clinical practical experience
• Preferred 2-3 years in Care Management, discharge planning, or home health care coordination

• Active and unencumbered Registered Nurse license in the state of Illinois
• Certified Case Manager preferred
