QIDP Case Manager
UCP Seguin of Greater Chicago is seeking a dedicated and compassionate QIDP/Case Manager to join our team. In this role, you will provide essential advocacy and service coordination for individuals with developmental disabilities, ensuring each person's physical, social, emotional, and vocational needs are understood and met. As a leader of the interdisciplinary team, you will guide the development and implementation of Individual Service Plans (ISPs) and collaborate closely with families, service providers, clinical staff, and community partners.
ResponsibilitiesThe ideal candidate is highly organized, an effective communicator, and committed to empowering individuals with disabilities to achieve independence and community integration. Responsibilities include coordinating services, conducting home and program site visits, monitoring progress, ensuring regulatory compliance, providing crisis support, and maintaining accurate documentation. This position also requires flexibility, including periodic evening or weekend hours and participation in an on‑call rotation.
Qualifications- Bachelor's degree in social services, psychology, or related field (per DHS requirements)
- Minimum 1 year of experience working with individuals with developmental disabilities
- Valid driver's license and reliable transportation
- Ability to complete required QIDP training and certifications
Monday
- Friday 9 AM- 5 PM with flexibility, on‑call Saturdays & Sundays
Salary: $23 - $26.50 per hour
