UCP Seguin of Greater Chicago is seeking a dedicated QIDP/Case Manager to provide advocacy and service coordination for individuals with developmental disabilities, guiding ISP development and collaborating with families, providers, clinical staff, and partners.
You will coordinate services, conduct home and program visits, monitor progress, ensure regulatory compliance, provide crisis support, and maintain accurate documentation. Some evenings and weekends are required, with on-call rotation.
