WOUND CARE COORDINATOR – Full Time – Generations Oakton Pavillion, Des Plaines, IL
Generations at Oakton Pavillion in Des Plaines, IL, has an Immediate Opening for Full Time Wound Care Coordinator, RN/LPN.
Must Have Wound Care Experience
Salary of up to $95k, Depending on Qualifications/Certifications/Whether RN or LPN, Plus Full Benefits, including 401k w/Company Match, HSA, Teledoc, Choice of Insurance Plans, and much more!
The Schedule will be Monday-Friday. On-Call Clinical and Manager-On-Duty Rotations required. Must work 3 holidays per year (1 Major and 2 Minor holidays). May occasionally be asked to come in on a weekend if survey, or scheduling needs.
Wound Care Certification Preferred, but Not Required

Responsibilities- Plan, implement and set priorities for wound care and prevention according to residents needs
- Medication application to existing wounds
- Educate patients and their families to care for their own wounds
- Care for patient s surgical incisions, any pressure sores or any wounds caused from disease or injury
- Skin evaluation for pressure sore prevention
- Stocking the treatment carts
- Complete MDS, careplan
- Work with wound physician on weekly rounds
- Current RN or LPN license from the State of Illinois
- Wound Care certification (WCC) or VOHRA preferred
- Wound Care experience required
- Nursing experience at a SNF
- MDS and Care Plan experience a big plus
- Personable and professional with residents, families, and staff
- Maintain confidentiality of all residents’ information in compliance with HIPAA standards
- Must have compassion, understanding and tolerance for older adults
