Key Responsibilities:

• Provide comprehensive hospice and palliative care services in accordance with best clinical practices and patient goals of care.

• Collaborate with physicians, nurses, social workers, and spiritual care providers to deliver interdisciplinary care.

• Conduct patient assessments, develop care plans, and manage symptoms including pain, anxiety, and shortness of breath.

• Communicate with patients and families about prognosis, treatment options, and end-of-life care planning.

• Ensure accurate documentation and compliance with regulatory standards.

• Current Nurse Practitioner license in the state of Illinois.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $120,000-$125,000 on an annualized basis.

The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
