Role Overview

As a home health registered nurse case manager, you will lead and collaborate to provide management and delivery of patient care plans, supporting patients in Mount Prospect and surrounding areas. This full‑time position offers a first‑year success bonus of $10,000.

Responsibilities
  • Lead and collaborate in developing and delivering patient care plans.
  • Manage and supervise patient care, ensuring timely charting and care quality.
  • Use tablets, smartphones, and EMR software to document patient care.
  • Travel approximately 20% of the day to patient locations.
Qualifications
  • RN license in the state where you work.
  • Graduate from an approved school of practical nursing.
  • At least one year of RN experience and clinical competence to deliver quality patient care.
  • Current driver’s license.
