Clinical Manager Registered Nurse; RN

Crystal Home Health Care is a mid‑size home health agency growing rapidly and expanding its footprint. We provide skilled nursing, physical therapy, occupational therapy, speech therapy, and social work services. We accept 15+ insurances along with Medicare and MMAI. • Oversee the delivery of clinical services, ensuring the highest quality of patient/client care in accordance with home health laws, policies and procedures
• Coordinate and manage a team of nurses, therapists, CNAs to ensure client and patient goals are met
• Provide daily supervision of all intake and scheduling teams to ensure the quality of services and maintenance of standards
• Review and investigate the clinical content, diagnoses, medical history, medications, fee status, and other procedures from the records of clients and their physicians
• Fill out the client’s re‑hospitalization referral/admission form sheet daily and submit it to the intake department with a recertification date
• Manage and coordinate client discharge
• Assist with the orientation of new professional staff to familiarize them with agency policies and procedures
• See clients in the home setting if needed
• Perform other duties as directed by the Clinical Director or President/CEO of the organization
• Collaborate with other departments and professionals to streamline operations

• Proven experience in Case Management/QA or supervising position
• Proficient in use of EMR systems and Microsoft Office Suite products;
  Well Sky/Kinsser experience a plus
• In‑depth knowledge of nursing best practices/procedures and KPI/KQIs of patient care
• OASIS documentation experience required
• Great knowledge of legal regulations and best practices in health care and home health
• Willingness to keep abreast of changing standards in nursing administration
• Excellent ability to lead and develop personnel
• Great communication and people skills
• Outstanding organizational, time, and stress management skills
• Aptitude in problem‑solving
• Valid nursing and CPR certification

Job Type: Full‑time

Pay: $95,000 – $110,000 based on experience
