Job Overview
Primary Care Physician (PCP) at Chen Med responsible for delivering high‑quality, value‑based care to a panel of up to 450 seniors.
Core Competencies- Accountability for outcomes:
Provide strong clinical care, cost‑effective treatment, and influence patient health outcomes. - Health coaching:
Set short and long‑term health goals and facilitate patient progress. - Simplifying for action:
Prioritize and clarify care plans to enable actionable behavior change. - Data‑driven performance:
Meet metrics such as admissions per thousand, appointment utilization, CGCAHPS, clinical gap closures, and cost measures.
- Provide independent acute and chronic care for older adults.
- Take full accountability for patient outcomes and seek specialist consultation when necessary.
- Coordinate with hospitalists and care teams for patients in hospital, SNF, LTC, or homebound.
- Lead care team comprised of medical assistant, care facilitator, and care coordinator.
- Engage transitional‑care teams, case managers, and other resources as appropriate.
- Participate in center management, cover for other providers, and assist in recruiting patients and providers.
- Attend risk and quality management programs, clinical meetings, and regulatory compliance sessions.
Job Duties
- Assessment, diagnosis, treatment, and management of acute and complex chronic health needs.
- Health promotion, education, and care coordination documentation.
- Active role in center operations and staffing coverage.
- Availability:
Open to walk‑in visits, prompt phone and message responses. - Service orientation:
Provide care as one would for own family. - Evidence‑based medicine and lifestyle approach.
- Leadership development and teamwork.
- Commitment to quality improvement and operational efficiency.
- Influence and compassion in patient and team interactions.
- Self‑care and physician wellness.
- Proficiency in Microsoft Office Suite.
- Willingness to travel locally, regionally, and nationally for training and recruiting.
- Fluency in English, with additional language requirement if specified.
US $221,141 – $315,915 (base annual salary). Compensation may include bonus or commission based on performance.
