• Collaborate to provide management and delivery of patient care plans in a team‑based environment.
• Support patients at home, ensuring timely and accurate charting.
• Self‑manage time and schedule, driving to patient locations approximately 20% of the day.
• Use tablets, smartphones, and EMR software for patient documentation.
• Participate in a comprehensive onboarding program and ongoing clinical education.
• Assist with 24/7/365 after‑hours care team duties when needed.

• Associate degree in physical therapy from an accredited college or technical school.
• Minimum one year of experience in an acute care and/or rehabilitative setting (home‑health experience preferred).
• Good‑standing license/registration as a Physical Therapist Assistant issued by the state of work.
• Current driver’s license; ability to drive 20% of the day to patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Proficiency with tablets, mobile phones, and EMR software.
• Successful completion of a drug screen (medical and recreational marijuana) is part of the background process.
• Consent to receive communication via phone or text by supplying a phone number.
