Healspace LLC and Talbot Health Services are seeking a Licensed Social Worker (LSW) to join our growing behavioral health team. This position plays a vital role in supporting individuals experiencing mental health disorders, substance use disorders, and co-occurring conditions through comprehensive care coordination, advocacy, resource linkage, and recovery support services.

Candidates must possess an active Ohio Licensed Social Worker (LSW) credential in good standing. Applicants without an active Ohio LSW license will not be considered.

Both full-time and part-time opportunities are available.

Position Summary

The Case Manager provides professional case management services to individuals receiving behavioral health and substance use disorder treatment. Working as part of a multidisciplinary treatment team, the Case Manager assists clients in identifying needs, overcoming barriers to care, accessing community resources, maintaining treatment engagement, and achieving individualized recovery and wellness goals.

This position collaborates closely with counselors, medical providers, nursing staff, peer support specialists, managed care organizations, referral sources, and community partners to ensure coordinated and effective service delivery.

  • Complete biopsychosocial and needs-based assessments related to housing, employment, transportation, healthcare access, financial stability, legal concerns, family support systems, and other social determinants of health
  • Develop, implement, monitor, and update individualized service plans based on identified client needs
  • Coordinate referrals and linkage to community resources, healthcare providers, government assistance programs, recovery support services, and behavioral health resources
  • Advocate for clients while promoting self-sufficiency, empowerment, and long-term recovery success
  • Assist clients in navigating healthcare, behavioral health, social service, housing, and criminal justice systems
  • Monitor treatment participation and collaborate with treatment providers regarding client progress and ongoing needs
  • Participate in multidisciplinary staffing meetings and treatment team discussions
  • Maintain accurate, timely, and professional documentation in accordance with OhioMHAS, Medicaid, CARF, and organizational requirements
  • Assist with discharge planning, aftercare coordination, and continuity of care services
  • Promote trauma-informed, recovery-oriented, and person-centered service delivery practices
Minimum Qualifications
  • Active Ohio Licensed Social Worker (LSW) credential required and maintained in good standing
  • Bachelor's Degree in Social Work from an accredited college or university required
  • Knowledge of behavioral health, substance use disorders, co-occurring disorders, and recovery principles
  • Strong assessment, case management, advocacy, documentation, and communication skills
  • Ability to maintain professional boundaries while building therapeutic and supportive relationships with clients
  • Valid Ohio driver's license, acceptable driving record, and reliable transportation
Preferred Qualifications
  • Experience in community mental health, behavioral healthcare, substance use disorder treatment, integrated care, or healthcare case management
  • Experience working within an OhioMHAS-certified treatment program
  • Knowledge of ASAM Criteria and levels of care
  • Familiarity with Medicaid-funded behavioral health services and documentation requirements
  • Experience working with housing resources, criminal justice populations, healthcare systems, managed care organizations, and community support agencies
  • Training in motivational interviewing, trauma-informed care, crisis intervention, or recovery-oriented systems of care
