A Brief Overview:

• The Medical Social Worker will function as an integral member of a collaborative and interdisciplinary team, using assessment, implementation, coordination, monitoring, and evaluation skills. Supports Nursing Services and other clinical and non-clinical ancillary services in assuring the continuum of care for patients, and in maintaining the quality-of-service delivery. The Social Worker of the Cross Care Continuum will also serve as a patient advocate.

What you will do:

• Participate with other staff members in developing and implementing program policies and procedures.
• Coordinate social work services and patient referrals between the program and other agencies providing social and educational services.
• Individual assessment, short term treatment, and referral of emotional problems.
• Crises intervention for emotional and situational problems.
• Group treatment for psychosocial problems.
• Family assessment, treatment, and referral for emotional problems
• Ongoing assessment of mental health needs in student population.
• Education of teachers, students, and guardian(s) and caregivers on topics related to mental health.
• Ability to work well in a team setting.
• Ability to implement philosophy and objectives of program.
• Ability to work cooperatively with sponsoring agency, school staff, students, and guardian(s) and caregivers.
• Ability to function effectively with other staff members as a team.
• Ability to teach classes and lead groups within the school setting.
• Ability to initiate relationships within the community and maintain liaisons with the lay and professional community.
• Ability to do public speaking relating to Health Center services and adolescent health.

What you will need:

• Education

  Masters Degree in Social Work,

• License

  Licensed Social Worker (LSW) - Illinois Department of Financial and Professional Regulation (IDFPR),
  required. or Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR),
  preferred

• Experience
  :
  Must be experienced working with adolescents,
  required
  .
• 1 to 2 years of social work experience in an acute care setting
  preferred
  . Spanish speaking
  preferred.
