A Brief Overview

This Program Manager, Quality & Care Transformation leads clinical quality innovation and performance improvement strategy and drives care transformation initiatives. The role supports the organization in developing and implementing performance improvement and clinical innovation projects, collaborating on metrics, data, tools and processes to monitor performance, coordinating accreditation compliance, and building strong collaborative relationships with operational leaders, physicians, and the Clinically Integrated Network.

Additionally, the role works closely with clinicians, office staff, CIN medical leadership, payors, and administrative data vendors to facilitate meeting quality improvement study submissions and targets, requiring autonomy and independence necessary for decision‑making that supports project speed and success.

What you will do
  • Develop organization‑wide reports, tools, and dashboards across platforms (Vizient, Epic, RLDatix, etc.) in collaboration with stakeholders.
  • Create high‑level executive summaries of system performance for public reporting (CMS Stars, Leapfrog, US News, etc.) with recommended improvement paths.
  • Participate in and contribute to value improvement activities by synthesizing quality, financial, supply, and other data to identify opportunities and actionable items.
  • Serve as liaison between stakeholders (clinical leaders, physicians, payers, HIM, etc.) to ensure accurate data elements for reporting.
  • Identify and facilitate creation of supportive reports to advance internal scorecard performance and achieve goals across multiple areas.
  • Analyze data to identify improvement opportunities based on internal goals, benchmarking, industry standards, and payor/regulatory rating systems.
  • Manage performance and process improvement projects at the hospital, region, and system levels.
  • Communicate and engage with key stakeholders to address issues, move projects forward, and define action steps when goals are not achieved.
Education
  • REQUIRED:

    Bachelor’s degree in a clinically‑focused area such as RN, OT, PT, pharmacist, health information technology, or other healthcare related degree.
  • PREFERRED:
    Master’s degree in a clinically‑focused area such as RN, OT, PT, pharmacist, health information technology, or other healthcare related degree.
Certification
  • Minimum Certification Requirements: CPPS/CPHQ, LEAN/Six Sigma Certification, and/or similar training.
Minimum Work Experience
  • 5 years of healthcare experience including data analytics or use of data to drive performance improvement.
Skills
  • Strong analytical skills to analyze complex datasets, identify trends, and resolve data quality issues.
  • Proficiency in process improvement methods and tools to evaluate, measure capability, and redesign operational and clinical processes.
  • Ability to interpret and display data in ways that highlight opportunities and engage leaders to develop care improvement tactics.
  • Experience with IHI, Lean, Six Sigma, and related methodologies.
  • Knowledge of data governance policies and procedures.
  • Analytical, problem‑solving, and issue‑resolution skills.
  • Excellent written and verbal communication skills to convey data issues and solutions to technical and non‑technical staff.
