In this role, you will serve as the primary link between Skilled Nursing Facilities (SNFs), patients, providers, and clinical teams, ensuring efficient communication and seamless care coordination. This position combines administrative support with relationship management and requires regular on-site engagement with partner facilities throughout the Chicago area.

This is a dynamic role within a growing market. Responsibilities, priorities, and scope may evolve as business needs change and new opportunities emerge. Regular travel and on-site visits (2-3 days per week) are required.

Your Impact

Key Responsibilities

  • Coordinate communication between SNFs, patients, providers, and care teams via phone, email, fax, Docusign and text.
  • Serve as a central point of contact to support timely information sharing and align clinician schedules for patient appointments at the facility.
  • Track, prioritize, and resolve outstanding documentation, orders, and follow-up items.
  • Navigate insurance requirements and facilitate timely processing of patient-related needs.
  • Maintain accurate, organized, and compliant patient records and documentation.
  • Support continuity of care by monitoring referrals and coordinating next steps with stakeholders.

Facility Partnership & On-Site Support

  • Build and strengthen relationships with SNF partners across the Chicago area.
  • Maintain a consistent on-site presence to support facility engagement and partnership development.
  • Identify, track, and follow up on referral opportunities.
  • Collaborate with facility staff to obtain required clinical and administrative documentation.
  • Ensure complete and accurate electronic health records to support patient care and operational excellence.
Minimum Qualifications
  • High school diploma or equivalent combination of education and experience required.
  • Be at minimum a good fit for the job, as determined by the Talent Bullseye assessment. Hanger will administer the Talent Bullseye as part of the selection process.
  • Preferred at least two to four years of customer service, administrative, or related experience required. Experience in a receptionist position or experience with medical front office procedures preferred.
  • Must have, or be eligible to obtain, a valid driver’s license and driving record within the standards outlined within Hanger’s Motor Vehicle Safety Policy and Procedures.
  • This role will be internally titled as a Patient Access Coordinator I or II based on experience.
Additional Success Factors
  • Strong interpersonal, oral (including telephone) and written communication skills.
  • Ability to key 30 words per minute with accuracy required.
  • Ability to work with handicapped individuals.
  • Understanding of Alpha filing system.
  • Understanding of medical reimbursement and terminology and an understanding of front office (receptionist) duties preferred.
  • Ability to use a computer, including Windows-based software and e-mail.
  • Ability to use all necessary office equipment, facsimile machines, calculator, postage machine, copier, etc.
