* Hours:

Monday-Friday, 8:00a-4:30p
** A Brief Overview:
** A frontline public health worker who is a trusted member of and/or has a deep connection or understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

* What you will do:

*** Assess and address patient’s needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient’s health outcomes (Social Determinants of Health).
* In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment.
* Assist patients with getting access to social services and other community-based resources.
* Follows up with patients via phone calls, home visits and visits to other settings where patients can meet.
* Maintain accurate documentation of patient contacts and document patient’s needs, action plan and outcomes in the medical record system.
* Represents the organization at various community events.
* Participate in continuing education, trainings and CHW organizations.

* What you will need:

**** Minimum Education Level
*** High school diploma, GED or equivalent.
* Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred
** Experience
* ** 2 Years Community-Based Experience.
* Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations.
* Ability to effectively engage in conversations, motivate and encourage patients to take steps to maintain and improve their health.
* Creative thinker with ability to problem solve and brainstorm solutions.
* Ability to work collaboratively as a part of a patient care team.
* Ability to work independently with minimal supervision.
* Leadership and community education experience.
* Basic computer skills required.

Experience with EPIC EMR preferred.
* Fluency in Russian, Spanish, Urdu, Hindi, or Arabic a plus.
* Patient Advocate Certification Preferred
* Valid Illinois Driver’s License Required
