Hourly Pay Range

$21.08 - $31.62 – the hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

A Brief Overview

A frontline public health worker who is a trusted member of and/or has a deep connection or understanding of the community served. This trusting relationship enables the CHW to serve as a liaison, link, or intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

What you will do
  • Assess and address patients' needs in the area of housing, food insecurity, transportation or other non-medical areas that influence health outcomes (Social Determinants of Health).
  • In partnership with patient care teams, provide support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment.
  • Assist patients with accessing social services and other community-based resources.
  • Follow up with patients via phone calls, home visits and visits to other settings where patients can meet.
  • Maintain accurate documentation of patient contacts and document patients' needs, action plans and outcomes in the medical record system.
  • Represent the organization at various community events.
  • Participate in continuing education, trainings and CHW organizations.
What you will need

Minimum Education Level
  • High school diploma, GED or equivalent.
  • Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred.
Experience
  • 2 Years Community-Based Experience.
  • Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations.
  • Ability to effectively engage in conversations, motivate and encourage patients to take steps to maintain and improve their health.
  • Creative thinker with ability to problem solve and brainstorm solutions.
  • Ability to work collaboratively as a part of a patient care team.
  • Ability to work independently with minimal supervision.
  • Leadership and community education experience.
  • Basic computer skills required. Experience with EPIC EMR preferred.
  • Fluency in Russian, Spanish, Urdu, Hindi, or Arabic a plus.
  • Patient Advocate Certification Preferred.
  • Valid Illinois Driver's License Required.
