The position supports patients in Skokie and surrounding areas and includes a $5,000 first year success bonus for key achievements during your first year.
The successful candidate will collaborate with the care team to manage and deliver patient care plans in a team‑based environment.

- Associate degree in physical therapy from an accredited college or technical school
- Minimum one year of experience in an acute care and/or rehabilitative setting preferred (home health experience preferred)
- License/registration as a Physical Therapist Assistant in good standing, issued by the state in which you work
- Current driver’s license and ability to spend ~20% of the day driving to/from patient locations
- Commitment to consistently meet critical deadlines for charting
- Self‑management of time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
