• Conduct comprehensive psychosocial assessments.
• Participate in care planning and resident care conferences.
• Advocate for residents' rights and preferences.
• Provide emotional support to residents and their families.
• Assist with admission, discharge, and transition planning.
• Coordinate referrals to community resources.
• Help residents and families navigate financial, legal, and social service programs when appropriate.
• Identify and address concerns related to resident adjustment, grief, loss, abuse prevention, behavioral health, and family dynamics.
• Maintain compliance with federal, state, and facility regulations.
• Collaborate with healthcare professionals to create a safe, supportive environment for residents to achieve their highest practicable level of physical, mental, and psychosocial well-being.

• Required Education and Experience
• High School diploma or equivalent.
• Two years’ experience in a social services role, preferably in a health care setting.
• Preferred Education and Experience
• Bachelor’s degree in social work or similar field.
