Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team;
Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence;
Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks;
Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR;
Work independently, but travel to patient homes within assigned territory
