Overview
The salary range for this position is $40.50 - $64.80 (Hourly Rate). Placement within the salary range is dependent on relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

The Clinical Nurse Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsibilities- Provides education, guidance, and support to patients/families to help manage therapeutic regimens and self-care activities for maximum independence.
- Facilitates access to rehabilitative services within available patient resources; responsive to internal and external customer needs.
- Assesses patient and family understanding of management of therapeutic regimens and self-care activities.
- Develops an individualized plan of care based on nursing diagnoses/problems, considering patient culture, age, and developmental level; sets goals.
- Implements and continually evaluates the plan of care and patient/family response.
- Provides patient/family education to manage prescribed therapeutic regimens.
- Coordinates with the team and scheduling to patient rounds.
- Coordinates discharge planning to facilitate transition to next level of care.
- Educates the team regarding patient s insurance benefits.
- Reviews patient progress for medical necessity and guideline compliance against benchmarks and functional goals.
- Communicates plans to referring agencies to facilitate a smooth transfer process.
- Interfacing with Admissions and PFS to verify/obtain discharge status as needed.
- Assures ongoing certification for continued stay; obtain pre-certification/authorization as required by payer for special services.
- Communicate effectively with patients and families using age-appropriate, language-accessible approaches and inform them of health status and progress.
- Provide clear, concise, and timely documentation of nursing services and case management discussions.
- Collaborate with internal and external case managers and coordinate services to optimize resource use for discharge planning.
- Serve as communication link between patient, family, and interdisciplinary team.
- Participate in education and training sessions; attend external continuing education programs and present information to staff.
