Job Title

Registered Nurse (RN) Case Manager – Harvey, Illinois

Job Summary

Responsible for achieving positive patient outcomes and managing quality of care across the continuum of care. Acts as a patient advocate, collaborates with the care team to develop and execute care plans, coordinates transitions between settings, and maintains communication with providers and families.

Responsibilities
  • Manages transitions of care, discharge, and post‑discharge follow‑up for patients admitted to high‑volume hospitals.
  • Establishes trusting relationships with patients and their caregivers.
  • Collaborates with clinical staff to develop and execute care plans, and reports variations to PCP/Transitional Care Physicians, implementing corrective actions.
  • Builds relationships with preferred acute and post‑acute providers, directing referrals as needed.
  • Coordinates social services, case management functions, and discharge/home‑planning processes with related departments and facilities.
  • Keeps PCPs informed of patient conditions via email, DASH, HITS, or other communication tools.
  • Explains the case manager role to patients/families, provides engagement and education, and facilitates conferences to review goals.
  • Serves as patient advocate, addressing advanced care planning, treatment goals, and advance directives.
  • Refers complex cases to social workers, counseling services, and reports suspected abuse as required.
  • Obtains EMR access at priority facilities, maintains clinical and progress notes, and provides timely documentation.
  • Participates in surveys, studies, special projects, and concurrent medical record reviews as assigned.
  • Investigates adverse events, performs staff education on resource utilization, and promotes efficient clinical resource use.
  • Evaluates services from admission through discharge for appropriate utilization and patient satisfaction.
  • Communicates with physicians regularly during hospitalization and assists in cost, case, and outcome management.
Additional Role-Specific Duties
  • Acute Case Manager – Focuses on inpatient vs. observation status, patient safety, functional status, medication management, and coordination of discharge to lower levels of care.
  • Community Case Manager – Provides telephonic/outpatient visits, disease‑management monitoring, medication review, health education, and coordination of care plans; visits may include evenings and weekends.
  • Skilled Nursing Facility Case Manager – Includes community case‑manager duties plus visits to SNFs, validation of care level and discharge plans, and coordination with PT, social work, and payors.
  • Transitional Case Manager – Combines acute and community case‑management responsibilities.
Qualifications
  • Registered Nurse (RN) licensure required.
  • Strong interpersonal and communication skills with diverse constituencies.
  • Critical thinking and autonomous work ability.
  • Proficiency in monitoring, assessing, and recording patient progress; ability to plan and evaluate care plans.
  • Knowledge of nursing case‑management theory, clinical documentation, and community health services.
  • Organizational and coordination skills; ability to communicate technical information to non‑technical personnel.
  • Proficiency with Microsoft Office Suite (Excel, Word, PowerPoint, Outlook) and other relevant software.
  • Willingness to travel locally, regionally, and nationwide up to 10% of the time.
  • Fluency in English; bilingual preferred.
Compensation

Hourly: $36.90 – $52.70. Final compensation depends on experience, education, geographic location, and other relevant factors. Eligible for bonuses or commissions.
