Staff Nurse (LPN or RN) - Part-time Opportunity at Pine Crest

As a Staff Nurse (LPN or RN) at Pine Crest, you will play a vital role in providing exceptional care to our residents, helping them regain their fullest potential of health and functioning. Responsibilities:

• Provide individualized care to residents, developing and implementing personalized care plans in collaboration with our multidisciplinary team

• Conduct initial and ongoing assessments of residents' conditions, identifying changes and making adjustments to care plans as needed

• Collaborate with our medical director and care team to monitor and update resident progress, communicating with physicians and families to ensure seamless care

• Set discharge dates and plans, coordinating with outside services to ensure a smooth transition back home

• Participate in multi-disciplinary team meetings to discuss resident care and develop strategies for improved outcomes

Requirements:

• Current LPN or RN licensure in the state of Illinois

• 0+ years of experience in a healthcare setting (education and training also considered)

• Ability to work part-time hours, with flexibility to adjust schedule as needed
