InHome Therapy – Licensed Home Health Physical Therapist

InHome Therapy is seeking a compassionate, motivated, and licensed Home Health Physical Therapist (PT) to join our growing team. You’ll provide one‑on‑one care to geriatric patients in their homes in and around Hoffman Estates, helping them regain mobility, reduce pain, and improve their quality of life. • Deliver in‑home physical therapy services
• Evaluate patient mobility, strength, and function to develop treatment goals
• Implement effective rehabilitation and therapeutic exercise programs for adult and geriatric patients
• Educate patients and families on recovery plans and preventive care
• Maintain accurate and timely home health care documentation

• Active PT license and BLS CPR certification

Compensation is paid per visit. Evaluation visit rates range from $60–$95, with additional visit types at varying rates. Final compensation is based on experience, qualifications, and internal equity.
