At Future Care, our Social Workers play a critical role in advocating for our residents and improving access to health care services within our facilities. They provide support through crisis intervention and promote good health and wellbeing for our residents. We provide clinical supervision to ensure that Future Care standards and quality are delivered at the highest level of competency.

The salary above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately and paid pursuant to the relevant program schedule. All employment offers will consider such factors as overall experience, job‑related qualifications, location, certifications/training, etc.

• Coordinate medically related social services to assist residents in maintaining or improving their ability to manage everyday physical, mental, and psychosocial needs.
• Assist in meeting the psychosocial needs of residents, their families, and significant others as necessary in coping with effects of illness, disability, treatments and stay in the facility.
• Address advance directives and assist in determining appropriate medical decision maker.
• Provide individual, family, and group services designed to maximize psychosocial well‑being.
• Promote actions by staff that maintain or enhance each resident’s rights and dignity, recognizing individuality.
• Assist in developing an interdisciplinary plan of care for residents.
• Provide support and grief counseling as indicated.
• Oversee and coordinate the discharge planning process to all other settings.

• Bachelor’s or master’s degree in Social Work.
• Experience in skilled nursing or sub‑acute health care setting preferred.
• Current, valid Maryland Social Work license.
• Ability to function as a member of an interdisciplinary team.
• Excellent interpersonal skills to relate to residents, families, and community agencies.
• Ability to prioritize, organize tasks to meet deadlines, and handle distractions.
• Ability to navigate and document in an electronic health record system.
