Franciscan Health Olympia Fields Campus 20201 S Crawford Ave Olympia Fields, Illinois 60461 The Social Worker II collaborates with individuals, families, and groups to address psychosocial challenges through a process of assessment, diagnosis, treatment, and evaluation. As part of a multidisciplinary care team, this position works directly with patients and their caregivers to resolve personal crises and support a therapeutic milieu to cope with chronic, acute, or terminal illnesses.

The Social Worker is also responsible for overseeing the general case management and care coordination of assigned patients.

• Assess the psychological status of patients related to the patient’s illness and environment.
• Carry out social evaluations and interventions based on evaluation findings.
• Provide information to patients or families and caregivers and community agencies.
• Provide patient and family-focused therapeutic crisis counseling.
• Continuously update case manager.
• Maintain clinical records on all patients referred to social work.

• Required Master’s Degree Social Service
• 2 years Acute Care Hospital Preferred
• 2 years Mental Health or related Community Healthcare Preferred
• Licensed Social Worker (LSW) – State Licensing Board
