Responsibilities
As a RN MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process.
Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
Applicants must have an unencumbered State of Illinois R.N. License, be a graduate of an accredited nursing program and hold a C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting, and (1) year of experience as a C.N.A. are preferable but we are willing to invest in the right candidate.
