Family Home Health Network is seeking a dedicated Hospice Nurse to provide compassionate in-home end-of-life care across the Chicagoland area.
You will perform initial assessments of patients and their families, develop and revise plans of care, and monitor nursing needs while delivering excellent customer service. This role requires an Illinois RN license, CPR, and the ability to work every other weekend (Sat-Sun 7am-7pm).
