Registered Nurse; RN - Hospice

Graham Healthcare Group is seeking a registered nurse (RN) for a hospice role located in Lincolnwood, Illinois.

The position involves collaborating with an interdisciplinary team to develop and implement patient care plans, documenting care in EMR systems, and consistently meeting charting deadlines. Duties include traveling to patient locations to deliver care, managing shift schedules, and supporting family‑centered care initiatives.

• RN license in the state of employment
• Associate degree or higher from an accredited nursing school
• Two years of RN experience, hospice experience preferred
• Current driver’s license; ability to spend ~20% of the day traveling to patient sites
• Proficiency in EMR software, tablets, and mobile devices
• Excellent time‑management and scheduling skills
• Proficiency in Russian or Spanish is a plus
