The successful candidate will coordinate nursing care by collecting and analyzing assessment data to determine outcomes and develop a care plan in collaboration with the patient and family, and other team members.
You will identify interventions to meet expected outcomes, implement them, evaluate progress, and document the process while addressing the unique needs of patients across the age spectrum.
