Position Summary:
We have an exciting opportunity to join our team as a LPN - NYU Langone Dialysis Center - Mineola, Days. In this role, the successful candidate Responsible for using his/her clinical experience to plan the delivery of safe, therapeutic, age appropriate patient care through planning, implementation and evaluation over a scope of practice. With the patient and family central to the care, the LPN incorporates the unique characteristics and needs of each patient and family member into the care delivery process.
Works collaboratively within a multidisciplinary health team to promote wellness and optimizes patient outcomes. Performs multiple clinical and clerical duties as required. Complies with established policies, procedures, rules and regulations as they pertain to the hospital as well as federal state and local regulations. Diligently observe all HIPPA guidelines. Performs all other duties as it relates to specialty.
Job Responsibilities:
Perform other duties as assigned. Completes documentation reflecting multidisciplinary coordination. Documents teaching/learning needs of patients and families while on program and in preparation for discharge. Evaluates and documents patients?
