Job Summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Lyons, Illinois. This full‑time position will support patients in Justice, Bellwood, and surrounding areas. The role includes a $10,000 first‑year success bonus.

As a home health registered nurse case manager, you will lead and collaborate to manage and deliver patient care plans. You will be responsible for charting, meeting critical deadlines, and providing care through a team‑based environment.

• RN license in the state you work
• Graduate from an approved school of practical nursing
• One year of RN experience with the clinical competence to deliver quality patient care
• Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations
• Commitment to consistently meet critical deadlines for charting
• Ability to self‑manage time and schedule
• Demonstrated experience with tablets, mobile phones, and EMR software
