MDS Coordinator RN or LPN – Full-Time – Generations at Applewood, Matteson, IL

Generations at Applewood in Matteson, IL, has an immediate opening for a MDS Nurse (RN or LPN). Salary for RN is $90k-95k depending on MDS certification and experience. Salary for LPN is $79k-$85k also depending on MDS certification and experience.

• The MDS Coordinator is responsible for collaborating with the Interdisciplinary Team
• Plan, organize, develop and implement the MDS assessments and care plans
• Update records and assessments, ensuring information is consistent with patient care plans and that scoring is accurate
• Accountable for accurate MDS scoring

• Current RN or LPN licensed from the State of Illinois
• American Association of Nurse Assessment Coordination (AANAC) certification preferred
• RAC-T Certified preferred
• Knowledge of Minimum Data Set (MDS), ICD-10 Coding
• Two (2) to three (3) years of applicable experience as a MDS Nurse
• Must be CPR Certified
• Experience in a long term care setting
