Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant to support patients in Westchester, IL and surrounding areas. The position is located in Maywood, Illinois and offers a $5,000 first‑year success bonus.

The Assistant will collaborate with the care team to manage and deliver patient care plans, provide support to patients in home health settings, and meet charting deadlines.

• Associate degree in physical therapy from an accredited college or technical school (required)
• Minimum one year of experience in an acute care or rehabilitative setting (preferred)
• Previous home health experience (preferred)
• Licensed or registered as a Physical Therapist Assistant in the state of employment (good standing)
• Current driver’s license and ability to drive approximately 20% of the day to patient locations
• Strong time‑management and scheduling skills
• Experience with tablets, mobile phones, and EMR software
