Medical Records Clerk

Position Summary: The Medical Records Clerk is responsible for managing and safeguarding patient medical records to ensure accuracy, confidentiality, and accessibility. This role involves data entry, scanning, filing, retrieval, and release of medical records in accordance with legal and organizational policies, following all HIPAA and organizational guidelines regarding patient privacy.

Key Responsibilities
  • Organize, scan, and file medical records and documents into the electronic health record (EHR) system.
  • Retrieve patient records as requested by providers, staff, or authorized third parties.
  • Process requests for release of information in compliance with HIPAA and state regulations.
  • Ensure medical records are complete, accurate, and up to date.
  • Bill, track, and reconcile medical records fees.
  • Maintain confidentiality and security of all patient information.
  • Assist with audits, chart reviews, and quality assurance activities.
  • Respond to internal and external requests for medical documentation in a timely manner.
  • Follow office protocols for record retention, destruction, and archiving.
  • Perform clerical duties such as faxing, mailing, and data entry as needed.
Education
  • Minimum:
    High School diploma or GED
  • Preferred:
    Associate’s Degree
Work Experience
  • Minimum:
    At least 2 years of experience in the healthcare industry
  • Preferred:
    Physician Primary Care and Hospitalist billing experience
Required Knowledge, Skills & Abilities
  • Strong attention to detail and organizational skills
  • Proficiency with EHR systems (e.g., Epic, eClinical

    Works, Athena Health)
  • Familiarity with HIPAA compliance
  • Excellent communication and customer service skills
  • Ability to maintain confidentiality and handle sensitive information
  • Comfortable working with computers, scanners, and office equipment
  • Physical Requirements:
    • Ability to sit for 6+ hours
    • Ability to type at a computer for 6 hours
    • Ability to lift 25 lbs.
