About the Role
Graham Healthcare Group – Residential Home Health and Hospice – is hiring a Home Health Physical Therapist to serve patients throughout Morton Grove, Illinois and surrounding areas. This full‑time, 40‑hour a week position is based in the Chicago, Lincolnwood, and nearby regions.
Responsibilities- Collaborate with a team‑based care model to develop and deliver patient‑specific care plans.
- Provide physical therapy services in patients’ homes, ensuring effective management of care and documentation.
- Maintain accurate electronic charting and meet critical deadlines for chart completion.
- Use tablets, mobile phones, and EMR software for patient management and communication.
- Adhere to a flexible scheduling model and support after‑hours care as required.
- Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
- Licensed Physical Therapist in the state of employment.
- Minimum one year of health‑care experience.
- Current driver’s license and willingness to drive approximately 20% of the workday.
- Strong time‑management skills and ability to self‑manage schedule.
- Experience with tablets, mobile phones and EMR software.
- Upward earning potential through a competitive per‑visit pay model.
