Hourly Pay Range

$56.44 - $70.58 – The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

  Advanced Practice Provider (APN/PA) – Primary Care – Family Medicine
• Location:
  
  199 W. Rand Rd, Mount Prospect, IL
• Full Time/Part Time:
  Full Time, 40 hour week
• Hours:
  
  Monday – Friday, between the hours of 7:00am – 5:00pm
• Rotating Saturday's, 8:00am – 12:00pm
• Rotating Call – Approx. one week/year

Exciting opportunity to join Endeavor Health in the Primary Care Department. Under the direction of the collaborating physician(s), the provider offers care to patients. The provider obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures, interprets results, and instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. The provider maintains records and writes progress notes on status, treatments, and procedures.

The provider follows patients through their course of treatment in the clinic and hospital setting, collaborates with physicians and the health care team to achieve optimal patient outcomes, and prescribes pharmacologic and non‑pharmacologic therapies as appropriate. The provider demonstrates evidence‑based practice, participates in the development, implementation, and evaluation of standards of practice, assesses learning needs of patients and families, employs interventions to promote health, and conducts comprehensive health assessments, differential diagnosis, and treatment under the protocol in the direct management of acute and chronic illness and disease.

The provider promotes wellness and preventive care across various settings for individuals, families, and communities, works autonomously and in interdisciplinary teams as resource and consultant, positively contributes to safety outcomes and high‑quality patient experience, participates in development and evaluation of service line goals, supports the strategic plan of the organization, and adheres to all Endeavor Health policies and procedures.

• License:
  Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required;
  Federal DEA and Illinois Controlled Substance License required
• Education:
  
  Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience:
  
  Minimum of 2 years APN/PA experience in Primary Care setting required
