* Summary:
** Reporting to the Behavioral Health/SUD Treatment Manager, this position is responsible for assisting patients with substance use disorder and co-occurring disorders for the purpose of providing strength-based recovery and case management services. This position helps to remove barriers and obstacles, assists with practices of harm reduction, and links patients to resources in the recovery community. Promotes the mission and philosophy of providing quality health care and related services to the medically underserved.

* Primary Responsibilities:
*
* • Ensures all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times.
• Acts as a role model by utilizing individual recovery experiences, advocacy, problem-solving, coping techniques, self-help strategies. Utilize empathy, support and concern to empower patient increasing confidence and self-esteem.
• Maintains a commitment to the achievement of quality health care services by maintaining patient rights, safety, privacy, confidentiality and excellent customer service philosophy.
• Conducts interviews with patients, their families’ and other individuals involved in the patient’s treatment
• Brief Intervention – engages patients showing risky substance use behaviors to connect with, link and coordinate treatment services and recovery support.
• Referral to Treatment – provides referrals to brief therapy or additional treatment for patients who screen in need of additional services.
• Conducts case management services for patients within community standards to set and accomplish specific goals. Focuses on clinical, psycho-social, education, financial and post inpatient residential follow for after care and Medication Assisted Treatment (MAT).
• Promotes and educates patient on relapse prevention and recovery-based strategies and tools.
• Conducts cravings screening, schedules and monitors attendance for routine scheduled MAT appointments.
• Participates in care management. Is fully engaged and integrated as part of the patient care team. Provides input into care plans, and alerts the care team of the psycho-social issues complicating health conditions.
• Secures needed release(s) of information, and other required documents for Substance Use Disorder services. Keeps records related to care activities secured, assuring records are accessible to medical/behavioral health providers as needed.
• Provides a trauma informed culture of safety to aid patients in their recovery process.
• Applies principals of recovery, stages of change and person-centered planning.
• Establishes and maintains open and positive communication in all aspects of job performance.
• Provides assistance and support to patients in crises; communicates with appropriate individuals and treatment team regarding changes, and or critical situations with patients.
• Performs other duties as assigned.

* Knowledge, Skills and Abilities Required:

* Education:
*
* • High School Diploma or completion of a GED program;
Bachelor’s degree, in counseling preferred
*
* Experience:
*
* • Experience as a recipient of substance use services and to be in recovery for two or more continuous years.
• Experience in navigating complex SUD, co-occurring disorder(s), and/or non-substance addictive disorder(s) treatment services
• Self-identify as having a SUD, co-occurring disorder(s), and or non-substance addictive disorder(s) with a substantial life disruption and share their recover story in supporting others.

* Skills:

* • Basic data entry skills, basic typing, filing, organizational skills.
• Ability to handle workload in stress filled environment without producing errors.

* Knowledge, Skills and Abilities Required:
*
* • Knowledge of patient care charts and patient histories.
• Knowledge of patient evaluation and triage procedures.
• Knowledge safety, environmental, and/or infection control methods.
• Knowledge of HIPAA regulations and compliance requirements.
• Knowledge of computer systems and applications, such as Microsoft Office 365
*
* Certifications:
*
* • Certified Alcohol and Drug Counselor (CADC) or development plan
• Michigan Certified Recovery Coach (MCRC),
• Certified Peer Recovery Mentor (CPRM)
• CPR
