Physical Therapist (PT) Lead – 13 Week Contract
Location: Northbrook, IL 60062
Position Type: 13-week contract
Experience
Required:
1+ year Home Health (HH) experience + HCHB (Homecare Homebase) experience required
Territory Coverage: Chicago team & Lake Forest region
Schedule: 12-hour day structure as needed (home health visits; schedule varies by patient census)
Licensure: Must meet Illinois PT licensure requirements
Position Summary & Responsibilities:- Provide in-home physical therapy evaluations and treatment plans
- Manage patient caseload across Chicago and Lake Forest territories
- Document visits and care plans using HCHB system
- Coordinate care with interdisciplinary home health team
