This position supports patients in Northbrook, IL and surrounding areas and offers a $5,000 sign‑on bonus. Eligible CNAs collaborate to manage and deliver patient care plans within our team‑based environment.

• Provide direct patient care and support under the supervision of nursing staff.
• Assist with daily living activities, comfort care, and monitoring of patient status.
• Document and communicate patient changes to the care team.
• Use the company’s tablet and mobile device to record care data and schedule visits.

• Certified Nursing Assistant (CNA) certification in the state of employment.
• High school diploma or equivalent (preferred).
• Hospice experience (preferred).
• Current driver’s license and ability to travel approximately 20% of the day to patient locations.
• Completion of a pre‑employment drug screen, including medical and recreational marijuana testing.
