Home Health Wound Nurse LPN (LPN).

Location:

Evanston, IL. Eligible for a $10,000 first‑year success bonus.

• Collaborate with the care team to manage and deliver patient care plans.
• Provide wound management and home health nursing services for patients in Evanston and surrounding areas.
• Maintain accurate and timely charting, meeting critical deadlines.
• Use EMR software, tablet, and mobile devices to document care and communicate with team members.
• Drive up to 20% of the time to patient locations.

• LPN license in the state of employment.
• Graduate from an approved practical nursing school.
• Minimum two years of LPN experience.
• Must possess WCC certification.
• Current driver’s license and reliable vehicle for driving to patient sites.
• Strong time‑management and self‑management skills.
• Prior experience with tablets, mobile phones, and EMR software.
• Successful completion of a background check and drug screen.
