Northbrook Health and Rehab, a leading long-term care and rehabilitation facility, is seeking a compassionate and dedicated Registered Nurse (RN) to join our team. As an RN, you will provide high-quality, patient-centered care to residents in a supportive, team-oriented environment. Our goal is to enhance the quality of life for individuals requiring long-term care, rehabilitation, and support, while maintaining their dignity and comfort.
ResponsibilitiesDirect Patient Care: Monitor and assess patients' health conditions, administer medications, and provide treatments as prescribed by physicians.
Care Planning: Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized care plans for residents.
Patient Advocacy: Serve as a liaison between residents, families, and healthcare providers, ensuring residents’ needs are met and their concerns are addressed.
Documentation: Maintain accurate, up-to-date records of residents’ health status, treatments, and progress.
Emergency Response: Respond to emergencies, providing immediate care and coordinating with physicians, specialists, and other healthcare team members.
Supervision: Supervise and mentor nursing assistants, LPNs, and other support staff to ensure high standards of care are met.
Compliance: Adhere to all healthcare regulations and facility policies, maintaining a safe and effective environment for residents and staff.
Current and valid Registered Nurse (RN) license in Illinois.
Strong clinical assessment skills, with the ability to recognize and respond to changes in patient conditions.
Excellent communication, organizational, and interpersonal skills.
Ability to work effectively in a collaborative, team-based environment.
Compassionate, patient-focused, and detail-oriented with a strong sense of integrity and professionalism.
CPR and BLS certification.
