Northbrook Health and Rehab, a leading long‑term care and rehabilitation facility, is seeking a compassionate and dedicated Registered Nurse (RN) to join our team. As an RN, you will provide high‑quality, patient‑centered care to residents in a supportive, team‑oriented environment. Our goal is to enhance the quality of life for individuals requiring long‑term care, rehabilitation, and support, while maintaining their dignity and comfort.

Key Responsibilities
  • Direct Patient Care: Monitor and assess patients' health conditions, administer medications, and provide treatments as prescribed by physicians.
  • Care Planning: Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized care plans for residents.
  • Patient Advocacy: Serve as a liaison between residents, families, and healthcare providers, ensuring residents' needs are met and their concerns are addressed.
  • Documentation: Maintain accurate, up‑to‑date records of residents' health status, treatments, and progress.
  • Emergency Response: Respond to emergencies, providing immediate care and coordinating with physicians, specialists, and other healthcare team members.
  • Supervision: Supervise and mentor nursing assistants, LPNs, and other support staff to ensure high standards of care are met.
  • Compliance: Adhere to all healthcare regulations and facility policies, maintaining a safe and effective environment for residents and staff.
Qualifications
  • Current and valid Registered Nurse (RN) license in Illinois.
  • Strong clinical assessment skills, with the ability to recognize and respond to changes in patient conditions.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work effectively in a collaborative, team‑based environment.
  • Compassionate, patient‑focused, and detail‑oriented with a strong sense of integrity and professionalism.
  • CPR and BLS certification.
