Hospice Registered Nurse (RN) Case Manager – Full-Time | Northbrook IL

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Northbrook area.

- Monday–Friday, 8:30 AM – 5:00 PM
- One weekend rotation every 12 weeks and 1 summer and 1 winter holiday
- Territory:
Northbrook, IL and surrounding communities - $80,000 – $97,000 annually (based on experience)

- Manage a dedicated hospice caseload as the primary RN Case Manager
- Perform patient assessments, admissions, and individualized care planning
- Deliver skilled nursing care focused on comfort and dignity
- Coordinate care with physicians and interdisciplinary team members
- Conduct routine visits and adjust care plans as needed
- Provide end-of-life support, including family education and pronouncement
- Supervise Home Health Aides in compliance with regulations
- Graduate of an accredited RN program
- Current CPR and First Aid certification
- Hospice or home health experience preferred
- Strong clinical judgment and communication skills
- Valid driver’s license and proof of insurance
