Position: Registered Nurse (RN) - Home Health - $75K-100K per year

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Northbrook, Illinois.

Job Overview

Role:
Home Health RN (Case Manager) – Ongoing, 40 hours per week, 8‑hour shift. Eligible for a $10,000 first‑year success bonus and a competitive pay‑per‑visit model.

Responsibilities
  • Lead and collaborate to manage and deliver patient care plans.
  • Provide case management and clinical oversight for patients in Northbrook and surrounding areas.
  • Document and chart patient information, meeting critical deadlines for charting.
  • Coordinate with clinical educators, preceptors, supervisors, and schedulers to ensure quality care.
Qualifications
  • RN license in the state of employment.
  • Graduate of an approved nursing school.
  • Minimum one year of RN experience and clinical competence to deliver quality patient care.
  • Current driver’s license; willingness to drive approximately 20% of the day to patient locations.
  • Ability to self‑manage time and schedule.
  • Experience with tablets, mobile phones, and EMR software.
