Job Overview

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice care. We are hiring an Admission Registered Nurse (RN) to provide end‑of‑life care to patients in their homes in the Northbrook, IL area. The role includes clinical assessments, individualized care planning, and collaborative teamwork within an interdisciplinary environment.

Responsibilities
  • Deliver compassionate, patient‑centered care in a hospice setting.
  • Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders.
  • Educate and guide patients and families through the hospice admission process, setting expectations and explaining the hospice philosophy.
  • Apply strong clinical judgment and critical thinking to facilitate timely admissions and ensure accurate, thorough, and compliant EMR documentation.
  • Initiate evaluations to other disciplines when indicated and work independently within the assigned territory.
  • Travel to patient homes within the assigned territory (approximately 20% travel).

Qualifications
  • Active Registered Nurse (RN) license in the state of employment.
  • Minimum two years of RN experience, with hospice experience preferred.
  • Current driver’s license and ability to spend approximately 20% of the day traveling to patient locations.
  • Successful completion of a drug screen as part of the background process.
