Residential Home Health and Hospice is seeking an experienced Hospice Registered Nurse to work in our Northbrook, IL location.

• Collaborate with the care team to manage and deliver patient care plans.
• Provide direct patient care and support in Northbrook, IL and surrounding areas.
• Document care accurately and meet charting deadlines.
• Travel up to 20% of the working day to patient locations.

• Registered Nurse license in the state of employment.
• Associate degree or higher from an accredited nursing school.
• Minimum 2 years RN experience with hospice care required.
• Valid driver’s license and ability to drive to patient sites.
• Strong time‑management and self‑management skills.
• Proficiency with tablets, mobile phones, and EMR software.
