We are seeking a compassionate Hospice Registered Nurse (RN) for after‑hours, PRN work in the Northbrook, IL area. The RN will collaborate with the hospice care team to manage and deliver patient care plans, ensuring high‑quality, patient‑centered support throughout each shift.

• Provide nursing care to hospice patients in a team‑based environment
• Develop, implement, and update care plans in collaboration with interdisciplinary staff
• Document patient status and interventions accurately and in a timely manner using EMR
• Coordinate care with patients’ families, physicians, and ancillary services
• Maintain compliance with state and federal hospice regulations

• RN license in Illinois
• Associate degree or higher from an accredited School of Nursing
• Minimum of two years of RN experience, with hospice experience required
• Valid driver’s license and willingness to spend approximately 20% of the day driving to patient locations
• Commitment to consistently meet charting deadlines
• Self‑management of time and schedule
• Experience with tablets, mobile phones, and EMR software
