Full‑time, 40 hours per week

• Collaborate with multidisciplinary team members to deliver and manage patient care plans in the home setting.

• Certified Nursing Assistant certification in the state of employment.
• High School Diploma or equivalent education.
• Minimum of one year of healthcare experience.
• Current driver’s license and ability to drive approximately 20% of the day to patient locations.
• Self‑management of time and schedule.
• Demonstrated experience with tablets, mobile phones and electronic medical record (EMR) software.
