Job Summary
Residential Home Health and Hospice is looking for a Hospice Registered Nurse (RN) to provide compassionate, high-quality care to patients in Northbrook, IL and surrounding areas. This 40‑hour per week position offers a comprehensive benefits package and an opportunity to impact patient outcomes in a supportive team environment.
Responsibilities- Manage and deliver patient care plans per hospice protocols.
- Provide bedside and home‑based care for patients in Northbrook, IL and surrounding areas.
- Maintain accurate and timely charting to meet critical deadlines.
- Self‐manage time and schedule, ensuring coverage for 24/7/365 after‑hours care as needed.
- Collaborate with clinical educators, preceptors, and supervisors to enhance clinical practice.
- Use tablets, smartphones, and electronic medical record (EMR) software for documentation and communication.
- Registered Nurse (RN) license in the state of employment.
- Associate degree or higher from an accredited school of nursing.
- Minimum of two years RN experience; hospice experience preferred.
- Driver’s license with ability to travel approximately 20% of the day to patient locations.
- Strong commitment to meeting charting deadlines and managing workflow independently.
- Proficiency with tablets, smartphones, and EMR systems.
- Proficiency in Russian or Spanish is a plus.
- First‑year success bonus up to $10,000.
