Post-Acute Network (PAN) Nurse
ALIYA Healthcare is seeking a knowledgeable and driven Post-Acute Network (PAN) Nurse to support patient placement, care coordination, and managed care relationships across our skilled nursing and post‑acute network. As a PAN Nurse, you will serve as a clinical liaison between hospitals, managed care organizations, and our facilities to ensure appropriate patient placement, strong outcomes, and efficient transitions into post‑acute care.
Key Responsibilities- Coordinate and facilitate patient transitions from hospitals and other settings into ALIYA Healthcare facilities
- Review clinical referrals to ensure appropriate placement within the network
- Collaborate with hospital case managers, discharge planners, and managed care organizations
- Support census development by identifying and securing appropriate admissions
- Monitor length of stay, readmissions, and patient outcomes across facilities
- Serve as a clinical liaison between facilities and payer/network partners
- Assist with authorization processes and managed care communication as needed
- Track and report trends related to referrals, denials, and network performance
- Build and maintain strong relationships with hospital and payer partners

- Active RN license required
- 2+ years of nursing experience in skilled nursing, post-acute care, case management, or hospital setting
- Experience with managed care, MCOs, or post-acute placement strongly preferred
- Strong understanding of Medicare, managed care, and post-acute services
- Excellent communication and relationship-building skills
- Highly organized with strong follow-up and documentation abilities
- Ability to work independently in a fast-paced environment
- Case management or care coordination experience
- Skilled nursing or hospital discharge planning experience
- Experience working with multiple facilities or a post-acute network
